The Bianchi 111 Cyclone Hip Holster that will work for your 586 is MPN:12694 Right Hand, and MPN:12695 Left hand. These are for the 4-4.2 inch barrel model. If you have the 6 inch barrel the right hand holster is MPN:12696, and the left hand is MPN:12697. This holster will not fit the S&W CTG in .38 spc.

Purchased this holster for a 3" GP 100 Wiley Clapp it fit fine only issue is with stiffness of bely loop which should be easily resolved. I was looking for a dual position holster and had experience with the cyclone and liked the design.
